For report status declaration, please use the latest form PSZ 19:16 (Pind.1/13). You may find the file in the attachment. The form is slightly different to the previous version PSZ19:16 (Pind.1/07), in terms of the wordings in declaration part and items in acknowledgement part.
Report checklist form is also attached. This form must be endorsed by supervisor before submit to the faculty, together with 2 copies of hardbound report, CD, and Turnitin reports.
